Simply put, care management changes lives. Simply having someone who is here to help you and guide you through your healthcare challenges can make a world of difference.

What is a care manager?
A care manager (or a Lead Care Manager) is a dedicated healthcare professional who is here to help you manage your care. They support, guide, and connect you with the services and extra care that you need. Here are some of the ways a care manager can help:
- Finding doctors / specialists and setting-up appointments
- Coordinating the medical care you receive from different providers
- Helping you understand your prescription drugs
- Helping with food services
- Helping with transportation
- Connecting with housing assistance and social services
Our Titanium care managers (or ‘Lead Care Managers’ as they are known under the CalAIM Enhanced Care Management Program) are a very special group of people. They build a strong rapport with their assigned members and will go the distance to get you the help and care you need.

Enhanced Care Management Program
Titanium Healthcare has been selected as a provider of care management services through CalAIM Enhanced Care Management (ECM) Program (formerly ‘Health Homes’). The program has been set-up to help Medi-Cal recipients get more coordinated and person-centered care. Members enrolled in ECM receive care management services and benefits at no extra cost.
How can I join?
Medi-Cal plans will contact members who qualify for the program. You can also ask your doctor or clinic about the program. You will stay enrolled in your Medi-Cal Managed Care Plan and continue to see the same doctors, but also gain an added layer of support. From homeless shelter to college
When Lead Care Manager, Amy, met ECM member, Dave*, he was homeless, living in a shelter and had no plan for his future. He was also in poor health and every day was a struggle. Amy set him up with a number of programs that helped him rebuild his life and connect with his family and faith. Dave now has his own apartment, sees his PCP regularly and is working towards his college degree. We’ve got your back Dave.

Life is now in focus
Mrs. Johnson* was in need of some new eyeglasses but could not afford a pair from Medi-Cal and, in any case, could not wait weeks for them to arrive. Titanium Lead Care Manager, Isabel, was able to help Mrs. Johnson apply for a program called New Eyes. After securing measurements and a prescription, Isabel was able to submit an application and her new glasses arrived quickly. “I’m seeing a whole new world!”– Mrs J.

Even if you don’t qualify for free care management services under CalAIM, you can still get help. Some of our patients will qualify for care management with their Medicare plans. Call your local Titanium clinic for more details.
For Hospitals, Health Plans and IPA’s
- Identify and manage comprehensive needs for Medi-Cal members through ‘whole person care’ approaches and social drivers of health
- Improve quality outcomes, reduce health disparities, and transform the delivery system through value based initiatives, modernization, and payment reform
- Make Medi-Cal a more consistent and seamless system for enrollees to navigate by reducing complexity and increasing flexibility.
Titanium Healthcare participates in two main CalAIM programs:
1) The Enhanced Care Management (ECM) program
The ECM program focuses on the Social Determinants Of Health (SoDOH)
2) Community Supports (CS) Program
The CS program focuses on the homelessness and housing needs for the homeless population.
How can hospitals and IPAs get involved?
When hospitals and IPAs become a partner with Titanium Healthcare they can benefit from the CalAIM initiatives in many ways, including, but not limited to:
1) Providing long-term housing options for the homeless patients
2) Addressing social issues for high utilizers like food insecurity issues
3) Reducing occupied hospital beds “ED and Inpatient” with homeless members awaiting safe discharge
4) Reducing occupied hospital beds “ED and Inpatient” with SMI members awaiting transfer
The ECM and CS programs are a long-term solution, so it is very beneficial for the members and the healthcare systems like hospitals and IPAs to partner with ECM providers like Titanium.
