Hundreds of Medicaid Patients in Yakima County Now Eligible for Extra Healthcare Coordination Services

Titanium Healthcare brings new care coordination services to unhoused, those with complex medical conditions, and other vulnerable residents through Washington Health Homes Program

YAKIMA, Wash. – September 28, 2026 — Titanium Healthcare has received approval to expand into Yakima County to help individuals with complex medical needs, including people experiencing homelessness, connect with healthcare and community services.

The expansion comes as health plan partners identify a need for additional care coordination services for members eligible for the Washington Health Homes Program. There are approximately 280 eligible members awaiting services in the region, according to the organization.

The company is working with health plan partners, including Coordinated Care, to help address the need. The organization also expects to serve eligible members through Community Health Group of Washington, which has a number of eligible members in need of care coordination services.

“Expanding into Yakima County gives us an opportunity to support individuals who face some of the greatest challenges accessing healthcare,” said Gray Miller, president of Titanium Healthcare. “Our goal is to help members connect with the medical care, community resources such as housing and support they need to improve their health and well-being.”

The company currently serves members in Pierce, King and Snohomish counties. The Yakima expansion brings the organization’s Washington service area to four counties.

The Washington team includes six care coordinators and one program manager. Dinorah Medina, Titanium Healthcare’s newest staff member, will support the organization’s efforts in Yakima County.

“Our job is to help patients with all the challenging ‘Social Determinants of Health,’ or non-clinical barriers that keep people from getting the care they need,” said Medina. “Simply put, we make sure the patient understands their plan of care, they get to and from appointments with their doctors and specialists, have enough medications to comply with their plan of care, follow an appropriate dietary regime, and have consistent checkups to make sure they stay on track”

Through the Washington Health Homes Program, eligible members receive support coordinating healthcare and connecting with services that address their individual needs. For people experiencing homelessness or managing multiple medical conditions, care coordination can help them navigate a complex healthcare system and access appropriate resources, especially housing.

The expansion reflects its commitment to supporting Medicaid health plan members and healthcare partners throughout Washington state.
