Director of Education and Quality Assurance

Jacob leads Titanium’s efforts to implement effective training across all Titanium programs. Unifying Training with a Quality Assurance function, Jacob drives performance excellence across the organization.

Jacob’s background is as an accomplished healthcare and education professional dedicated to enriching the lives of individuals with diverse needs. With a proven track record of fostering positive outcomes and expanding services, Jacob brings expertise in DHCS regulations, healthcare programming, and LGBTQI+ community advocacy. Throughout his career, he has consistently demonstrated excellence in providing oversight and leadership to teams, developing and implementing innovative programs, and ensuring compliance with regulatory requirements.
Jacob’s decision to join Titanium Healthcare was rooted in a profound alignment with the organization’s mission to revolutionize healthcare delivery. He finds inspiration in Titanium’s commitment to delivering comprehensive care to complex members and is driven to elevate the standard of care through innovative approaches and unwavering dedication. Nichole Trejo

Quality and Clinical Training Specialist

With 15 years of nursing experience and 10 years of honorable service in the United States Army, Nichole has built her career on leadership, compassion, education, and service to others. For the past ten years, Nichole has served in nursing leadership and educational roles, where she discovered her true passion for teaching, mentoring, and developing the next generation of healthcare professionals. Nichole takes pride in creating supportive learning environments where individuals feel empowered, included, and confident in their growth and abilities.

Nichole’s journey with Titanium began as a Nursing Care Coordinator, where she quickly demonstrated strong clinical knowledge, leadership, and dedication to both members and staff. Through proven results and a commitment to excellence, Nichole advanced into the role of Clinical Trainer, where she now specialize in staff development, clinical education, health plan audits, workflow compliance, and exception documentation. She is highly detail-oriented and passionate about ensuring quality standards are met while helping teams succeed operationally and clinically.

Nichole is known for her strong communication skills, ability to connect with others, and commitment to advocacy for both patients and staff. She genuinely enjoys collaborating with diverse teams and understands different learning styles so that everyone feels supported, included, and set up for success. As a leader, Nichole believes in leading with integrity, accountability, and encouragement while building high-performing teams that consistently deliver positive outcomes.

Above all, she is passionate about giving back to her community, empowering others through education, and ensuring every member and employee feels heard, valued, and supported throughout their healthcare journey.

Cecilia Bruzzone

CalAIM Pro Trainer

Cecilia is a dedicated CalAIM Pro Trainer with over eight years of experience in the healthcare field. She brings valuable hands-on experience as a former Lead Care Manager (LCM), where she supported members in navigating complex healthcare systems and accessing essential services. In addition to her direct member support, Cecilia has played an active role in assisting with shadowing and onboarding new hires, helping them build confidence and competence in their roles.

Cecilia is passionate about empowering both members and staff—ensuring individuals feel supported as they navigate their health and healthcare journey.

Sandra Martinez

CalAIM Pro Training Leader

Sandra leads training and staff development initiatives across Titanium’s CalAIM programs, with a focus on strengthening team performance, supporting workforce growth, and creating meaningful impact for both staff and members. Having started her journey at Titanium Healthcare as a Lead Care Manager (LCM), Sandra brings firsthand operational experience and a strong understanding of the challenges and opportunities within care management.

Sandra’s background includes experience in behavioral therapy for children on the autism spectrum, along with specialized work in community mental health and advocacy. She earned her Bachelor’s degree in Sociology from University of California, Riverside and her Master of Public Health with a focus in Community Mental Health. She is also a certified Domestic Violence Advocate and Certified Mental Health First Aid Instructor. Sandra is passionate about equipping individuals and teams with the tools, confidence, and support needed to navigate complex situations while fostering healthier workplace environments.

Samantha Garcia

CalAIM Pro Trainer

Samantha has been working in healthcare for approximately five years, beginning her career as a Medical Assistant before joining Titanium Healthcare. After transitioning into the Lead Care Manager (LCM) role, Samantha spent a little over a year supporting members through care coordination, advocacy, and resource connection.

During her time as an LCM, she also served as a designated shadower, helping train and support new team members as they learned the role. Being able to guide others and share knowledge is what sparked her interest in joining the CalAIM Training Team. Samantha is passionate about creating a supportive learning environment, helping staff feel confident in their roles, and contributing to the growth and success of the team.

Karissa Chan

Vice President, People Operations

Karissa Chan is a passionate human capital strategist specializing in transforming People Operations to align with business strategy, particularly for high-growth organizations. With a background in organizational development and a commitment to enhancing healthcare efficiency and accessibility, Karissa brings experience in overseeing People Operations for teams operating Medicaid care management and social services programs.

Prior to joining Titanium, Karissa held HR leadership roles across healthcare, technology, and financial services industries, where she successfully scaled teams and implemented company initiatives to foster inclusive, high-performing cultures.

Karissa’s academic background in Sociology, with a focus on social determinants of health from the University of California, Irvine, is the foundation for her professional mission to address healthcare challenges. She is currently an MBA candidate at UCLA Anderson School of Management and is a SHRM Certified Professional (SHRM-CP).

Kate Vicars-Cosio

People Operations Specialist

As the People Operations Specialist at Titanium Healthcare, Kate focuses on enhancing employee experience and organizational effectiveness through recruitment, onboarding, and employee engagement initiatives. Her journey began as a dedicated public health professional, driven by a passion for health promotion, education, and maternal and child health. Graduating with her Bachelor’s in Public Health during the height of the COVID pandemic, she recognized a unique opportunity to apply her education to make a meaningful impact on her community.

Kate initially served as the Lead Care Manager, working directly with members at the forefront of care, where she developed a deep understanding of the challenges faced by individuals in accessing health services. After a year and a half, she pursued a Master’s in Human Resources Management from Southern New Hampshire University, combining her public health background with HR expertise.

Since transitioning into HR, Kate has concentrated on fostering employee wellness while aligning it with organizational health. She believes that a healthy workforce is foundational to high performance, advocating for comprehensive public health initiatives that support employee well-being. Kate is committed to integrating public health principles into human resource practices, driving initiatives that benefit both employees and the organization. With her unique blend of skills, she strives to create a workplace culture that prioritizes health and wellness, ensuring that employees thrive both personally and professionally.

Joe Jordan, MD, MBA

Chief Medical Officer

Dr. Jordan has over 20 years in leadership of health care. He began his career as a Hospitalist Physician. After spending time in hospitals and with hospitalist physicians, Dr. Jordan realized that the processes for patient care could be vastly improved. Characterized by lots of paper and redundant effort, the changing of these processes became his mission. Dr. Jordan then lead the Kaiser Mid-Atlantic division as a Hospitalist Physician Executive. Dr. Jordan was most recently Chief Medical Officer for Intercede Health, in Houston, TX, where he led 150 physicians in activities ranging from Hospitalist to Intensivist and Utilization Management.

Dr. Jordan has also worked extensively on the payor side, both private and Medicare, to improve care management processes facilitating the payor- provider relationship.

Dr. Jordan has a passion for excellence and efficiency. In particular, the space of chronic patient management is an area where doing the right thing for the patient creates value across the board and saves significant expense.

Dr. Jordan graduated from Medical College of Virginia, earned his undergraduate from the University of Virginia and his MBA from George Washington University.

Rosie Robinson

Compliance Specialist

Rosie Robinson, MHA, Compliance Specialist, is an enthusiastic, driven, and compassionate healthcare leader with 20 years of experience in the healthcare industry. She began her healthcare career as a customer service representative, then decided to go to school to become a nurse, but realized that anatomy was not her niche. She graduated from the University of Phoenix with a Master’s in Healthcare Administration.

After graduating, she became a Utilization Management Supervisor and transitioned to a Utilization Management Compliance Auditor. In this role, she realized her passion for healthcare laws, rules, and regulations. After looking back at her career choice, she realized that she was always fascinated with the law since she enjoyed watching television shows such as SUV, Law and Order, and Blue Bloods!

Her goal is to create a culture of compliance at Titanium Healthcare where employees understand the importance of adhering to laws, rules, regulations, and internal policies and genuinely believe in doing the right thing.

Amy Agle

Director of People Operations

As the leader of People Operations, Amy is responsible for shaping a workplace culture that prioritizes both employee well-being and organizational success. With over 15 years of People Operations leadership experience, including roles across retail, manufacturing, food processing, and healthcare, Amy combines strategic insight with a people-first approach to drive engagement, growth, and operational excellence.

Holding an undergraduate degree in Physical Education and a Master’s in Management and Leadership, Amy brings a unique perspective on human potential and leadership. Known for being resilient, trusted, and authentically collaborative, she empowers teams to thrive through trust, advocacy, and a shared commitment to success.
